Collect Program Data

This form will allow you to schedule data collection for a cohort within your program. This involves filling out a Cover Sheet for the cohort, a separate survey that is taken once by the proctor or instructor to provide information about the cohort as a whole.

Select your program area below, enter the link to your Qualtrics survey, and click Get data collection links. This will create paired links to your appropriate coversheet and survey for use with one specific cohort.
